PROLAPSED INTERVERTEBRAL DISC (PIVD)

The contributor to Low Back Ache

Experiencing pain, numbness, and weakness is not a great sign of good vertebrae health. What if we say, a good physiotherapy evaluation and assessment with a timely diagnosis could avoid the aftermath of PIVD. But before we emphasise the significance of early physiotherapy treatment, let us first dive into what basically is PIVD and what causes it.

Prolapsed Intervertebral disc or PIVD is the herniation of the intervertebral disc. Talking about the typical structure, a vertebral disc is sandwiched between two vertebras. The Intervertebral disc is like a cushion that serves the purpose of shock absorption. Early degenerative processes, trauma and other factors can potentially lead to displacement or herniation of the intervertebral disc outside its boundaries. The result is the compression of nerves, ligaments present around the prolapsed vertebral disc.

The most common segment to be affected is the lumbar segment followed by the cervical vertebrae.

THE EVILS CAUSING PIVD

The presentation of symptoms is as per which segment is involved and whether the nerve is impinged or compressed. The following are the causable factors of PIVD.

  • Degenerative process: the nucleus pulposus gradually becomes less hydrated as the person ages.
  • Trauma: Any potential trauma could further up the chances of a prolapsed disc.
  • Congenital disorders as the presence of short pedicles in the vertebrae.

CLASSICAL PRESENTATION AMONG PATIENTS

The presentation of PIVD symptoms usually varies among different strata of individuals.

Talking about the symptoms in patients stuck with cervical disc herniation, they may suffer different horizons of symptoms as per which nerve root is compressed and impinged at a given level of the vertebra. The description of the type of pain people experience is usually burning or sharp shooting. Furthermore, nerve compression can cause tingling or numbness that may radiate down the limb.

The most commoner vertebrae in the cervical region are usually  C6-C7. Following is a glimpse of what type of symptoms can be presented in solitary nerve lesions.

  • C5: Pain present in shoulder, neck and scapular region. Numbness in the lateral aspect of the arm. Limb weakness during movements of shoulder external rotation, shoulder abduction, and elbow flexion.
  • C6: Pain present in shoulder, neck, and scapular region. Furthermore, the arm, forearm and hands also have pain. Limb weakness during movements of shoulder external rotation, shoulder abduction, elbow flexion forearm pronation and supination.
  • C7: The standard pain is present in the neck and shoulder. Numbness is experienced in the middle, and index fingers involving the palm region also. The elbow and wrist are the regions affected by weakness, which occurs during movement of forearm pronation and wrist flexion.
  • C8: Numbness present on the medial aspect of the forearm and hand. pain is present in the shoulder, neck and forearm region. movements like wrist extension, finger flexion, abduction and adduction cause weakness.

The lumbar disc herniation usually gives symptoms like back pain, that may radiate down the buttocks, thighs or legs.

  • L1: the person usually complains of sensory loss and pain in and around the inguinal area.
  • L2-L3-L4: The back pain usually radiates down the anterior aspect of the thigh and the medial aspect of the lower leg. Weakness is observed in movements like knee extension, hip adduction and flexion.
  • L5: pain in the back region radiating down into buttocks, lateral side of the thigh, calf and great toe. weakness is observed while hip abduction and knee flexion.

HOW WOULD YOUR PHYSIOTHERAPIST HELP YOU FIX YOUR PAIN?

You might find a lot of exercises and maneuvers being flooded all over the internet. But not all exercises are meant for you. every individual suffering from PIVD has a different causative factor and different biomechanical and muscular faults. So instead of ruining the prognosis, Let your physiotherapist do the work. So, a comprehensive evaluation and assessment would be the beginning. Your physiotherapist would learn about your comfort and the pain-provoking positions and then would tailor a program well-suited to fix your pain. The physiotherapy session would unfold in four phases which are usually Pain management, Stretching, and Chiropractic adjustments followed by strengthening of the required muscle groups. Other interventions can also be added as per the individual needs. Faulty vertebral alignments are also a problem-evoking condition. A proper chiropractic adjustment would help to bring the malaligned vertebra back to its normal alignment which is then reinforced by an appropriate regimen of strengthening.
